Update spouse full name

Sir/Madam,
I need to renew my passport since validity date expired. While doing that, I need to update: (1). New address which is different from the previous address. (2). Full name of my spouse. I am staying in Rajahmundry, Andhra Pradesh. We have POPSK here which is under the jurisdiction of Regional Passport Office Visakhapatnam. What is the procedure to apply for renewal.

My passport was issue from Ghaziabad (UP) PASSPOPRT Office in 2009, now it is going to expire after 6 months. Can I renew it from Visakhapatnam as I am working here presently. Please guide. Thank you.
